  • Birth Certificate/Marriage Certificate (proof of relationship to the applicant)

  • Bank Balance Certificate(around 1 Million pesos)
  • Bank Statement (at least 1 year)
  • If sponsor is an employee
    • Certificate of Employment
    • Income Tax Certificate
    • Salary Certificate
